Can you help out the diagnosis for a two year old boy suffering from dengue but the platelets were 400000 in count and the dengue report is positive ....boy was weak lean and looks like dehydrated moreover chills fever .....can anyone there for case study
diagnosis is clinically, on the basis of reported symptoms and physical examination. Diagnosis is according to patient have; fever plus two of these (nausea and vomiting, rash, generalized pains, low white blood cell count, positive tourniquet test, or any warning signs:{1)Worsening abdominal pain 2)Ongoing vomiting 3)Liver enlargement 4)Mucosal bleeding 5)High hematocrit with low platelets 6)Lethargy or restlessness 7)Serosal effusions} in endemic areas. >> treatment: Oral rehydration therapy to maintain urinary out put 1cc/kg/h if he could tolerate drinking and use paracetamol for fever; don't use ASA and NSAIDs because of risk of bleeding. Serial check of vital signs and Hb,Hct if he is unstable. Avoid nasogastric intubation, intramuscular injections. No need of antiviral drugs. Conservatively he will be better within 7 days. If platelets bellow 50000 use whole blood for compensation.
