A 34 years old man came to the ER due to neurogenic defects mainly in the lower limbs, and having severe lower back pain . His history indicated a long term of pain ,tenderness in the back and fever Investigations - >leuckocytosis , elevated inflammatory markers above normal and septicemia . What's your DDx?

Tuberculous osteomyelitis ,correct. But What made you think of malignancies?

Brucelosis osteomyelitis? Except septesemia all the symptoms come with it.
Osteomyelitis of spine is the most important one in this case. In my opinion the first step is imagining (MRI)
