Electrodes are placed on the scalp of the patient and an electric current is applied to induce a seizure under the effect of anaesthesia. There are two types of electrode placement; unilateral and bilateral. Bilateral include; bi-temporal, bi-frontal, and left anterior right temporal (LART). The most common and effective placement is the bi-temporal however it can cause disorientation which leads to an increased amount of care for the patient. Bi-frontal is as effective as bi-temporal but produces lesser disorientation in the patient. LART is as effective as bi-temporal and bi-frontal placements and produces lesser side effects. Unilateral includes right unilateral placement, also known as the d’Elia placement. Since the left hemisphere is dominant in most people, it is referred to as the right unilateral electroconvulsive therapy. Among bi-lateral and unilateral ECT, bi-lateral is more effective with more side effects and unilateral is less effective with more relapses. Types of placements: • Bi-temporal placement: Electrodes are placed on the temporal bone on each side of the head • Bi-frontal: Electrodes are placed on the frontal bone on each side of the head • Left anterior right temporal: One electrode is placed on the left frontal bone, while the other is placed on the right temporal bone. • Unilateral: One electrode is placed on the non-dominant frontotemporal area, while the other is placed lateral to the midline vertex on the non-dominant side.
