Definite peaked T wave. Hyperkalemia causes that, but SOB and vertigo aren't expected symptoms. What is his heart rate? Does his BP drop when he has the vertigo and SOB?
Look at the absent p wave and the small fibrillation waves in its place.
Big T wave and relatively big heart could tell us that could be cardiomiopathy and his symptoms are saying that he could have arytmia that is not seen in this ecg but could be cause of his vertigo
