This was due to corneal perforation. I have seen a few of these during my opthalmology rotation this year. They can be very very suttle. A patient complained of a scratching feeling in his eye(like when there is dust or a hair in the eye). I could not see anything with the naked eye. Only saw a small bulge upon examination with a slit lamp. (The patient I am talking about is NOT the one in the picture) Moral of the story. Always believe your patients and be thorough.
Believing in the patient is really crucial as it is proven with your case.. a patient is the one who knows his/her body best
