What is meant by Confusion Assessment method (CAM)?
Delirium is a state of mental confusion or a syndrome characterised by fluctuating or decreasing cognitive impairment along with behavioural abnormalities. It may includes the following features: Global cognitive deficit, Varying conscious level, Memory impairment, Strange behaviour, Disordered thinking, Disturbed perception, Mood swings. Confusion Assessment Method (CAM) is a standardized tool used for detecting delirium quickly and accurately. CAM has the sensitivity of >90% and specificity of 90-95%. Diagnostic algorithm of Confusion Assessment Method (CAM) includes: A. Acute onset and fluctuating course Evidence of acute change in mental status of patient form the baseline Did the normal behaviour come and go, fluctuate during the day or increase or decrease in severity? B. Inattention Does the patient have difficulty in focusing? becomes easily distracted? have difficulty keeping track of what is said? C. Disorganised thinking Is the thinking of patient disorganized or incoherent such as rambling speech/incoherent conversation, unpredictable switching of subjects and unclear or illogical flow of ideas? D. Altered level of consciousness Shown by any answer other than 'alert' to the following: -Overall how would you rate this patient's level of consciousness?: -Alert (normal) -Vigilant (hyperalert) -Lethargic (drowsy, easily aroused) -Stupor (difficult to arouse) -Comatosed (unarousable)
