How to diffrentitiate between crohn's disease and ulcerative colitis clinically?
Ulcerative colitis: 1-Patient presents with left lower quadrant pain and bloody diarrhea 2- Starts from rectum and goes only as far as cecum (with a continuous lesion) 3- Comprises mucosal and submucosal ulcers only 4- Main highlight is cryptic abscesses with neutrophils 5- Lead pipe sign is seen on imaging. Crohn's disease on the other hand: 1- Presents with right lower quadrant pain with non-bloody diarrhea 2- Can occur anywhere from mouth to anus and is in the form of skipped lesions (lesion then a normal part then another lesion) 3- It's a full thickness inflammation with knife-like fissures 4- Main highlights are creeping fat, cobblestone mucosa and granulomas 5- String sign is seen on imaging
