What is anaphylactic shock and how to approach patient with it?
Over-activation of immune system in response to an allergen causes the release of chemical mediators such as bradykinin (anaphylaxis). This causes the blood pressure to drop i-e anaphylactic shock. The best choice of treatment is epinephrine (to reduce anaphylaxis and restore blood pressure) A CPR maybe necessary if the heart stops pumping and the patient goes into severe cardiac arrest (along with epinephrine)
the physiology behind this briefly, that the allergen will cause an IgE mediated reaction which leeds to a direct degranulation of mast cells. Because of this histamin will be released --> H1 receptors on the bronchus gets activated and the airway will be narrowed H1 receptors are also on the endothel cells and this will be the reason why the blood will be not in the direct circulation, bc the permeability grows. Its an important step to ensure the blood pressure with epinephrin, but you also have to give antihistamin. (otherwise the patient does not respond to epinephrin) Antihistamin: Solu Medrol iv, or if the case is not severe: Suprastin po
