Volutrauma is a life threatening complication of mechanical ventilation characterized by overdistention of lung tissue or alveoli that damages the alveolar capillary membrane and leads to poor gas exchange. The lung injury occurs as a result of inflammatory response activation due to overdistention. It is also associated with extra-alveolar air that leads to complications like pneumothorax, pneumomediastinum, subcutaneous emphysema and pneumoperitoneum. Initially it was said to occur from large tidal volume but it can also occur in cases of normal tidal volume and significantly depends on the lung volume and compliance. The risk of volutrauma is increased in patients with Acute respiratory distress syndrome (ARDS) due to poor lung compliance and capacity.
