What is the reason for checking TPMT levels in ulcerative colitis?
To assess the patient's risk of serious side effects from treatment with thiopurines (e.g. azathioprine, mercaptopurine and thioguanine).
When a patient presents with an acute episode of ulcerative colitis. Management includes symptomatic relief of pain, anti-inflammatory and IV steroid therapy. Apart from these immunosuppressant use is also recommended. The commonly used are thiopurines. These include: azathioprine and metcaptopurines. These drugs when given are monitored. Thiopurines are metabolized by thiopurine methyl transferase enzymes, in absence of this enzymes giving these drugs can be fatal. Hence TPMT levels are checked before starting a treatment with thiopurines.
