Barrett Esophagus

It is complication of long standing reflux disease. After several years of GERD, normal epithelial lining changes from squamous to columnar epithelium. Men age more than 50 with chronic GERD (+5 years) with additional risk factors (nocturnal symptoms, hiatal hernia, smokers) should be screened. Patient with Barrett esophagus should have repeated endoscopy every 3-5 years to check for dysplasia or cancers. - If low grade dysplasi, repeat endoscopy 6-12 months. - If high grade, do radiofraquency ablation or endoscopic mucosal resection. - The usual rate of progression to cancer 0.5% every year. All patients with Barrett esophagus should receive PPIs.
