Auricular Hematoma or Cauliflower Ear

Auricular hematoma also known as cauliflower ear or boxer's ear is a deformity of the ear which is caused due to blunt trauma to the auricle and/or its surrounding tissue. This type of trauma is usually seen in individuals who participate in contact sports like boxing, wrestling etc, hence the name boxer's ear. The blunt trauma leads to the formation of a hematoma between the cartilage and the perichondrium. This disrupts the blood supply of the auricular cartilage which can lead to necrosis and infection. The resulting fibrosis is responsible for the development of a cauliflower ear. The clinical features include a swollen, warm and red ear which may or may not be accompanied by one or more of the following: difficulty in hearing, tinnitus, facial swelling, earache, headache. The aim of the treatment is to make an attempt at restoring the cartilage's blood supply. This is done primarily by draining the formed hematoma by making an appropriate incision and then applying a compressive dressing. Antibiotics are also given to prevent or treat infection. With prompt management it is likely to treat cauliflower ear but any delay can result in permanent damage.
