This clot was removed from the external iliac artery via a Fogarty's catheter. The catheter was advanced beyond the clot and the balloon was inflated. Upon pulling the catheter back, the clot dislodged from the vessel wall and was removed. External iliac artery is the primary source of blood to the lower limb. Its occlusion leads to limb ischemia which presents as pain aggravated by walking a particular distance and relieved by rest. Severe ischemia even leads to rest pain. Gangrene of the limb represents the end stage characterized by irreversible ischemic necrosis.
Frank gangrene and ischemic pain, the limb was cold and entirely pulseless
