Elderly patient is probably a fall risk. I wonder which end of the fork penetrated, as I can't really tell from this view. Wish there was another picture
I would think the dull end of the fork penetrated the back of her/his hand because the damage depicted in the picture shows that the entry wound is more neat or less "messed up" from the one you would get from the spikes of a fork.
