Nappy rash creams often contain steroid preparations and need to be used with caution. Unsupervised, excess use of such creams causes widespread absorption of steroids in the baby’s circulation and leads to Cushing’s Syndrome! Cushing’s syndrome is a disease characterized by elevated glucocorticoid levels in the body which not only cause the characteristic moon facies and truncal obesity, but also result in metabolic disturbances culminating in hypertension and diabetes mellitus. One such infant is presented here. This 11-month-old baby girl developed increased facial swelling and was also found to have high blood pressure and slight muscle wasting. She had been given nappy rash cream containing a potent steroid named fluticasone for 2 months. Source: Gillani S, Danish BA, Muhammad Shah SF, Hussain F, Aziz F, Iqbal A.
