Hey guys, I need your help. A patient has meningitis (unknown cause), headaches, high temperature (38-39° in the evening, in the morning it comes down, sometimes even to normal), he can't urinate spontaneously and feels weakness in his legs. MRI shows no changes on the brain. Ultrasound of both kidneys showed no damages too. What could be the case here?

The undulant nature of the temperature makes me want to question about travel to a P. falciparum endemic area, which could explain the leg weakness but other blood values needed. Otherwise, if the meningitis is certain, I would consider viral causes of meningoencephalitis

What treatment has he been started on so far ? Was the blood test redone since the 1st one was inconclusive ?
