Exam. You have to actually physically know what your baseline is for best practice. Too many HCPs have a hands off practice but it's inefficient and what If it is a patient with limited history or unrecorded info? Travellers? Immigrants? Being a doctor is more than just reading charts.

Both, there are some things you can diagnose almost solely with a physical and for other cases you need the history as well. The combination of both male the difference.
