Depends - is the person in diabetic ketoacidosis ? Is it diabetic type 1 or type 2 ? If type 2, normally they start with regular treatment like changing the person’s diet to a diabetic diet, increasing exercice. If it’s too advanced, they start them on medicament by mouth (pills such as metformin or januvia, or other options out there). Last resort is insulin SC, usually comes with a scale at the beginning to gauge the doses. Important to teach the patient about signs of hyper and hypoglycemia. Important to know ppl can become unconscious if blood sugar is too low or if it’s too high for too long. If patient is in DKA bring to emergency departement. Intensivists will start a protocol that includes IV insulin and IV dextrose and lots of fluids with frequent monitorings of vital signs and laboratories such as VBG and electrolytes (replacement of potassium frequently needed). That protocol lasts until the anion gap is closed and other laboratories are back to normal, usually they’ll transit them to a subcut insulin scale and a long action insulin daily or bid and monitor them for a couple days before they send them home with their new prescriptions and directives. Teach the patients, it can save their lives :)
