My sister who is pregnant with her second baby in her third month of pregnancy feels unbearable pain in one leg.

Make her see a doctor! (Esp if its red and larger, but id suggest doing that even without those symptoms) it could be deep vein thrombosis, which is a risk factor for lungembolism. Pregnancy is know to higher risk of thrombosis.
