In long standing stenosis, right heart failure may develop which leads to venous backup affecting the liver. This would be a long term complication.

Cause pulmonary edema and congestion leads to right ventricular failure and that affect venus rereturning to right atruim and accumulation of blood in inferior vana cava to back ti liver cause hepatomegally
