hypothyroidism why both t4 and TSH level decreased in case of secondary hypothyroidism

Secondary hypothyroidism means that there is some disease behind it which is interfering with the pituitary gland secretion of TSH (causes are usually tumors e.g craniophyrangeomas or thormbosis) and leads to low TSH. Low TSH leads to low stimulation of thyroid gland to secrete T3 and T4. That's why both TSH and T4 are low.
