The main dds will be - Tophaceous gout - look for uric acid levels and synovial aspirate in microscopy for negative birefringence. Pseudogout can be ruled out with X-ray look for calcinosis. RA usually spares the DIP joints. Xanthoma tuberosum can be ruled out with cholesterol levels.
